Envita™ Partner Program Claim Form

Thank you for using Envita in 2019. We are sorry Envita didn’t provide 3bu/ac yeild increase for you.
We hope you will try Envita again.

Before continuing to make a claim, please ensure ALL of the criteria below were met:

Program details:

  • Purchased and applied Envita prior to June 30, 2019 for use on corn
  • Applied within a registered state within the USA
  • Fertilized your field at recommended rates of nitrogen fertilizer (i.e. 100% of recommended N)
  • Applied Envita on Corn in-furrow or as a seed treatment at labeled rates and per use guidelines
  • Used at least 40ac of Envita and had the Envita in a defined space in a field
  • Left an appropriate Check within the same field (i.e. minimum of 5ac, side by side treated area, similar soil type and topography)
  • Used a harvest monitor to measure the yield of the Envita treated area and the Check area

To assist in the fulfillment of your claim, please fill in the following information no later than December 15, 2019.

All claims must be submitted no later that December 15, 2019. 

Azotic North America will let you know the status of your claim within two weeks of receiving your application. We may follow up with you by phone to collect further details.

  • Once your application has been approved, if you have chosen product reimbursement for 2020, we will contact you to discuss
  • Once your application has been approved, if you have chosen, $5/ac cash back, we will send you a check by February 15, 2020

If you would prefer product, we will ship new product to you by April 20, 2020.
